Children’s Care Management (CCM)

The Bowen Center’s Children’s Management provides services for children and youth from the ages of 3 to 20, who reside in Manhattan and/or the Bronx, who have active Medicaid, and have qualifying conditions, and medical and social needs including.

The program is expands services to an underserved population which includes children that are chronically ill, Complex Trauma, and Serious Emotional Disturbance.

Care management services have been proven to enable patients develop a more stable lifestyle by reducing their psychiatric hospitalizations and health-related hospitalizations, Mediate communication between providers (doctors, social workers, etc.) and the member, Assist with medical and mental health referrals and appointments for the member, Provide support by attending medical and other types of appointments with the member, link the member and family to variety of community referrals and resources, and much more to enable clients to maintain a more stable lifestyle.
